Referral

A majority of people who access our services are self-referrals and no doctor referral needed. We are also a registered member of the Family Services Employee Assistance Program. Please refer to your employer/plan to determine if we are contracted to provide services through your EAP.

We offer affordable multi-services for children, adolescents* and adults on several topics such as:

Family Services Association - Referral Picture of Man

If you are interested in bringing in a child under the age of 16, we would need to know if there are any custody arrangements in place. If you do not have sole custody, then we would require a signed consent form by both parents. These forms can be found under the Forms Section.

During the Intake Interview you will be asked basic questions about yourself and a brief description of your current life circumstances such that the intake counselor will be able to help you determine what we can do to help.

We also offer expanded service options that particularly pertain to youth, parents and family. Currently we offer:

  • Children Witnessing Violence Program which is nationally recognized to deliver service to children aged 7 to 14 who have been exposed to family violence, community/street violence or bullying.

  • Dads Making A Difference is a psychoeducational group designed to help fathers improve and maintain positive relationships with their children

  • Relationship Enhancement Program is designed to help couples build positive, healthy relationships.
